Drooping Brows: Brow Lift Approaches and How They Differ From a Temporal Lift
26 September 2026Under-eye bags are a common concern because they can make the face look tired. In some people bags are related to temporary swelling, while in others they become permanent as the fat around the eye bulges forward. In this article we explain the causes of under-eye bags and when lower eyelid surgery (lower blepharoplasty) may be considered.
Why do under-eye bags form?
The fat surrounding the eyeball is held in place by a thin membrane and muscle in front of it. With age these supporting structures can weaken, allowing the fat to protrude and create a bag. An inherited tendency can cause bags at a younger age. Salt intake, sleep patterns, allergies or certain health problems can cause temporary puffiness that is worse in the morning and improves during the day; the cause of such swelling should be assessed first.
Filler or surgery?
The appearance under the eye is not always caused by bags. A hollow at the junction between lower eyelid and cheek can make bags look more prominent. In people without marked bags but with hollowing and volume loss, filler may be considered in suitable cases. In people with marked fat bulging and excess skin, however, filler does not remove the bag and may even increase the look of puffiness. This distinction is made at the consultation.
How is lower eyelid surgery planned?
The aim of lower blepharoplasty is to reduce protruding fat or reposition it into the hollow and, if needed, remove excess skin. Depending on the case, the incision may be made just below the lash line or on the inner surface of the eyelid; with the inner approach there is no visible external scar. The appropriate approach depends on excess skin, lid tension and the state of the fat.
Recovery
Swelling, bruising and a feeling of tightness around the eyes are normal in the first days. General measures such as cold compresses and resting with the head raised may help. Noticeable swelling usually decreases within a few weeks, while the result may take months to settle fully. Seek medical attention without delay in case of changes in vision, severe pain or rapidly increasing swelling on one side.
Frequently Asked Questions
Can creams get rid of under-eye bags?
Bags caused by protruding fat do not disappear with creams. Creams can support skin care but do not correct structural changes.
Does lower eyelid surgery leave a scar?
With the inner-eyelid approach there is no visible external scar. Incisions below the lash line usually leave a fine line that becomes less noticeable over time; scar formation varies individually.
Can upper and lower eyelid surgery be done together?
In suitable people, upper and lower eyelid surgery can be planned in the same session. This is decided at the consultation according to general health and needs.
